Contractor Pre-Qualification Application Form

All contractors performing work in the public right-of-way, whether under contract or by permit, must have an approved and current Contractor’s Prequalification form on file with the Public Works Department of the City of Ashland.  The contractor must be preapproved to perform the work for which they are bidding/applying before a contract will be entered into or a permit will be issued. Prequalification's must be renewed biannually (September 1 to August 31 of the following year).

Any contractor or subcontractor applying for prequalification must have a current CCB license in good standing as well as an active City of Ashland business license. 

In the event that the entity actually performing the work is to be a subcontractor, the general contractor must fill out any necessary permit application form(s) stating that a subcontractor will be employed. The general contractor shall specify the name of the subcontractor and list the sub’s CCB license number on the permit application form. The subcontractor must also be prequalified to perform work in the public right-of-way.
