Under the City of Ashland's Wellness Program, employees enrolled in a qualified fitness facility can receive up to $120.00 per year reimbursement to offset the cost of membership. Reimbursements are paid twice per year during the months of January and July for a maximum of $60 per six-month period ($10/month maximum). Click below to download the Reimbursement form.
